Well, the latter is not the only option, but on the out-of-verse plot level it's most likely - if Emily wasted all her time on "not letting anyone else live either", that would be just too sad for Shouri's work.

Well, looks like she's being a bitch on purpose so she wouldn't have ties to anybody. Is that some sort of consideration toward others ? "I'm going to die, so best be alone so my passing won't make people sad" ?

And, yes, those look like scars.
Though the main one above the sternum seems weirdly far up for the heart...

Or rather a justification. Because it's not like, for example, Emily could expect a randomly met waitress to find out about her eventual demise and weep and a clumsy attempt to get her fired is the only solution. Unless she's

I just did a Google image search for 'heart surgery scar women' and this seems to be consistant.

Well, from the pics on the webpage, the scar should go down between her breasts, maybe even deeper.
