Because most youth is spent in the immortal Now, Emily. Unfortunately you don't seem to have that luxury.

I wonder if this story will involve the girls dragging Emily into enjoying the Now while acknowledging the probable future... Enjoying life while she can.

Judging by Emily's comment - "This could all end tomorrow" - it suggests not so much a terminal illness (whereby at some point in time the body deteriorates leading to eventual death) but a condition whereby something could trigger death over a matter of minutes / hours / days, and she's got to just attempt to live life while knowing that something could potentially trip the trigger at any time (which could tie into the story's name). Perhaps it could be she's had a major organ transplant and the pills are anti-rejection drugs?

However, we won't know for certain until she's motivated to open up to the gang...

And given todays update, opening up might be a ways off.
