That's what I thought too, at first. After re-thinking and commanding my brain to unsee the clownface (just like looking at art from Victor Vasarely), I think she's just showing her teeth with the corners of her mouth pointing downwards, trying to escape the suck.

Could be just me, but Empathy commenting on the the guys glistening with sweat seems so forced/awkward - like a line you'd find in a cheap porno. Is the writer is trying to make it sound sexy?

Which is really weird - considering the location (Heaven) and the source (a prudish looking angel).

Prudish "looking" may be the key word there, its been a theme that the angels are incredibly repressed sexually and basically latch onto anything remotely sexual they encounter.

It is probably suppose to be her gritting her teeth but

with the way she's discolored as a ghost, Her teeth aren't defined so with only the lines defining changes... what should be her open mouth, and teeth look like sad clown lips
