Well, modulo accidents.

I tend to assume that Alchemy's job is much like that of Pratchett's Death; she ensures the clean separation of the soul from the body, so it can go on to wherever it's supposed to go. (And she can tell you where that is, and give you a start in the right direction, just as a courtesy.) So she's not going to go round scything healthy material mortals, but if she finds a soul drifting away from its body but still not fully detached, out comes the scythe to tidy stuff up.
